Immunization Records and all previous Physical/Sports Forms can be found in your patient portal and can be downloaded/printed immediately!
To provide you with the best possible health care, it is important that our office receives a complete copy of your child's health records, including immunization records, and that you complete our release prior to your first visit. Please give the form to your child's previous healthcare practice, and have them forward a copy of your child's records via mail or fax.
If you would like a copy of your child's medical records, please fill out the form below and mail or fax the request to our office. Be sure to include the exact location you want records mailed or faxed. Please allow 30 days for medical records to be processed.
